Kakihara is a yakuza hitman and a sadistic character. Following the death of his boss, he scours Tokyo's underworld to find the culprit. In the process, Kakihara, known for his sadistic tendencies, crosses paths with Ichi, a mentally unstable and extremely violent killer. The bloodbath caused by Ichi attracts Kakihara's attention and the events turn into a bloody confrontation.
